Basically a web page that will run offline on any machine with a web browser that has input fields like name and email, when submit button is pressed the details are saved to a file or database. Preferably minimal installation of 3rd party plugins required.
Some solutions might include JavaScript libraries, flash or google gears for offline database.
Suggestions on other solutions welcome.
No design needed, simply the input fields and submit button.
