Hi We need someone to create the an application form in a testing environment; making sure that all work is developed to run on a Windows 2000/2003 server running Macromedia Cold Fusion (4.0 or greater) and Microsoft Access database.
You will not be provided with the control panel or database access or ftp.After creating the form you will have give instructions on how to connect to the existing access database.
The specs are given below :-
a) Development of a multi-step (multi-page) web-based HTML form that will emulate the 19-page PDF form. All form contents will be stored in a database for review. Quote is based on 355 form fields (unique stored data fields).
b) All form fields are required; if the user fails to fill out a form field they will be notified of any omissions or errors and required to go back and correct.
c) Upon completion of the web-based form, data will be used to create a completed PDF version that will be emailed to us.
Requirement :-
the form must be digitized with all data securely stored in a database that already exists on the site.
Must utilize e-signatures (please quote expense separately)
a progress indicator should be present as shown in the attached jpej
applicants should be able to create profiles, save their progress and return to the form
. a master administrator should have the ability to access and edit records.
