Email Form And Download Option  26.03.08

Need basic email contact form installed on my site. I’ll send you the file. You code it I’ll uploade it to my server. Must include Your Name: Your email: Home or Cell number: Inspection Address: Please type message here:

Also need a way for my clients to upload a report (word document) from either a MySQL database or server. Not sure how it would work so you’ll have to explain it to me. They would visit my site enter user name and password and once they hit submit they would have the option to save their report to their desktop.
