Import Csv In To Mysql Db  26.01.08

I need a csv file to be imported into a mysql database. The details are in the attached file.
The structure shown is from an existing db, but this import is into a fresh copy.
I will need about 6 more of these imports over the next few weeks, so you may bid on all of the jobs, one job, or some other solution for me to import the csv’s as needed.

Please ask about any concerns.
Quick work and good communication is a must!
