My website contact form is not working and I would like to add Validation options and Calendar options for the form to pick the date easily.and I would like to receive as an Excel file when it is submitted ,I am sending as an attachment my contact.php file and the file with codes. It will be helpful to use to convert excel file and you will see in my form HTML and jpg files how would I want to format for excel output. Forms must based on jpg files format including ingredients as much as possible.

3 form will be using excel output in same format like in scan1-2.jpg
all forms will have validation and spammer security and Pick date Java Calendar
Gas and Electricity Form will have Attachment Options as well
when they submit the form it will convert to excel and send it to my email automatically without notice the visitors.(like open or save the file)
