Joomla Multi Page Form 14.04.08
I need a form creation tool for Joomla 1.X that allows me to create one or two page forms.
For example on the form below I created some fields for page 1

The visitor will then select an item from the list box, complete the page 1 form and when they submit this page a custom created page 2 form will appear. That was created for the selected school.
The visitor can also come from another page and have a certain school selected.
When the user completes the page two form an email is sent to two or more addresses and the data needs to be recorded into a database.
Then the visitor comes back to the sample page above with all the page 1 fields completed from the first entry, then the visitor can select another entry without having to complete the page 1 fields.
Field types will need to be:
Text
Drop Down (display 2 to 10 items and allow single or multi-select)
Check Box
Text Area (comment - non HTML)
Radio Button
Field level validation will need to be:
Required
Not Required
