Need Form Created Asap 20.10.08
I need the following:
3 page or ’section’ form.
Form consists of a total of 19 yes/no questions.
5 questions in section 1.
If user selects “yes” for all 5 questions, download link appears for file #1.
If they answer “no” to any of the 5 questions, there is a next button to proceed to section 2.
In section 2 there are 4 questions.
If user answers ‘yes’ to all questions, download link appears for file #2. If user answers ‘no’ to any of the questions, a next button appears to go to step 3.
Step 3 is final section with 10 questions.
If user answers ‘yes’ to all questions, download link appears for file #3. If they answer ‘no’ to any of the questions, a download link appears to download file #4.
Screenshots attached but its pretty straight forward.
If easier to make 1 page form using ajax with sections appearing if necessary, that would be great too.
