Pdf To Database  30.05.08

1. We have a few interactive pdf forms that we need users to fill out then click submit and the data automatically gets stored in a mysql database on our web hosting company server

2. Once the pdf is filled out and users clicks submit want the actual pdf document to get stored on the file server too.

so we will have data from pdf stored in a database and we will have the actuall filled out pdf form document stored on the web host server too.

can you do one and or both and which? how much does this cost?
