Php Event Registration Script  25.08.08

I’m looking for someone to build me a simple event registration script that will use Paypal as the payment method.

These are the fields that will need to be collected from the user:

First, Last Name
Street Address
City, State, Zip
Email
Phone

Once they register, I’d like it to automatically generate a html reminder email that they get with the event details (including logo if possible)

There will need to be a way to view who has registered as well as confirm that payment has been completed successfully.
