Php Script: Contact Form 22.06.08
We need a Contact Form Script developed on PHP with options to use it as Feedback Form and Tell a Friend Option. A javascript link to “Feedback Form” & “Tell a Friend Form” is placed in the content page. Website visitor click on the link and a popup window is opened.
“FEEDBACK FORM” script -
Display “referer URL” and “referer page title” on the form
Rate our website: Excellant, Good, Average

EMail ID: (validate email id)

Select Department: (each department can have different e-mail id to which the message is forwarded)

Retrieve referer URL and referer page title and display it in the message
Display the IP number of sender, date and time in the message
“TELL A FRIEND FORM” Script -
Display “referer URL” and “referer page title” on the form

Friend’s Name: (make it possible to send to 5 friends)
Friend’s Emal: (validate email id)

Retrieve referer URL and referer page title and display it in the message
Send a copy of the message to “admin”. Include date, time, sender’s ip, referer url, referer page title, sender’s email id, name, friend list and their email ids in the copy to “admin”
Optional facility to disable sender’s message to avoid misuse of form and default message is send to the friends.
There should be Admin Control Panel to manage the options

We need bug free script. It should be unique and exclusively prepared for us.
