I need a form created within my existing website cctvproshop.com, this form is for employee to fill out an application for employement. Please use the attached application for all the fields. Once it is submitted it should email it self to a specified address and then bring the applicant to a confirmation page where I should beable to put a custom message instructing the applicant if applying for “sales position” please download the foolowing forms, sign and fax them fax to complete your application process. For all other applicants please allow 24 hours for our human resource department to review your application.
