First I want a login and registration page with basic info… and for course the registered user needs a way to edit his basic info and passwords etc…. recover lost password link etc…
next once logged in the user has access to the form mentioned above and the form auto populates the client information section from the general info provided by the registered user….
also on the title company section (name, contact and telephone) put a check box at the top so if the user checks it, it auto populates from the client information section….
Then finally once submit is hit the form e-mails to the admin and a confirmation copy to the user….
