I need a registration form completed for people to enter their details and an email get’s sent to one email address.

Reason for singing hope: e.g. supporting community, standing beside friends, …..
(room for a long sentence)
Chosen rehearsal venue: (circle or tick or whatever)
Wednesday 13th August 6:30 - 8:30 p.m. Lower Hutt venue to be advised
Thursday 14th August 6:30 - 8:30 p.m. Upper Hutt Heretaunga College Hall.
Friday 15th August 6:30 - 8:30 p.m. Piano Shop Plimmerton
Saturday 16th August 2:00 - 4:30 p.m. Wellington City venue to be advised

I am living in Wellington / I am travelling to Wellington for the weekend to sing from _______________.
I understand that in registering I am committing to attending the evening rehearsal and taking part in the SOS massed sing out on Sunday 17th August. I also understand that there will be no audience, we are all there to be the difference by singing out clearly and cooperatively as one heart under the direction of Simon Toyne, David Price, Paul Sojo and the leaders and choir of Kotuku. This is a gentle, inspirational massed chorus family of New Zealanders gathering with respect and dignity for each other to sing hope and record what that looks and sound like, not to be used for any political or personal gain. It is also a once in a lifetime freely given opportunity to learn from international choral directors on voice technique, sing in other languages and record a CD and DVD with them for the betterment of humanity.
I understand that in registering I am giving my permission to be filmed as part of the singing rehearsal and SOS massed sing out onto DVD, onto still photographs and recorded onto CD for all to be released as the SOS song of hope.
I acknowledge that:
Any funds generated from the sales of the DVD, CD, photographs and inspirational writings, or any other part of the long term effects of SOS will go into the Kotuku Music Academy Support Charitable Trust for the purposes of funding this event, the concert and any further ongoing SOS outcomes, and building Kotuku as an early intervention youth initiative.
All Copyrite including all the music and performance generated during and from SOS is owned by Kotuku Music Academy Support Trust.
I give permission / do not give permission for my personal contact details to be used for notifying me of further hope initiatives (e.g. the concert on 1st November final details).
