Submit Comments W/ User Accoun 07.04.08

I’ve got a project where a user (student) uploads a file which goes to another user (teacher) when the teacher logs into their account, they are able to see any files submitted to them.
(Keep in mind, this part has been fully completed and i can provide the files for you to work on them just adding the extra function)
IMPORTANT!!
!!What i need now is for when the teacher checks the work, they need to provide feedback on what the user had submitted,this could be done using a comment box or whatever. but it should automatically have reference to the teacher the comment came from and what file the comment is regarding for when the student looks at it
i have the MySQL tables regarding the files ready and i will provide them, any changes made to the files, just let me know which files have been changed
OK the most important part for this is, i need it a.s.a.p
considering theres not that much to do, im expecting a timeframe of about 2 hours
you can look over the files and see everything is pretty much ready for you
