Survey For Website 29.08.08
We need to build a survey tool on a page which we will provide with header and footer at www.elikaassociates.com containing all the standard features (multiple question types - which we need to be able to add and delete questions, graphical reporting, cross-tabulation, etc).
We need the ability to create a value for each question in turn the system would calculate the values and generate a report out of 100 lets say. It needs to have various permission levels, including a report only view.

Results in Real-Time
Ability to Browse Individual Responses
Geographical location or where the survey was taken
In addition on the same page we need to create a graph which updates after every survey and displays the appropiate value as calculated from each survey.
Needs to be programmed in PHP with AJAX. The survey must be well designed with css to be in theme with our website.
